The appointment isn't the treatment. The homework is.
You are here for forty minutes. The exercises are ten minutes a day for six weeks, and they are the part that actually changes anything. Any clinic that suggests otherwise is selling you attendance.
Pain that keeps returning — Backs, necks, shoulders, knees
After an operation — Following your surgeon's protocol
Getting back to a sport — On criteria, not on a calendar
Any change in bladder or bowel — Or numbness around the saddle area
Sudden weakness in a limb — Or pins and needles that spread
Chest pain or breathlessness — Emergency services, not a clinic
You get a number at the assessment — An estimate, said out loud, and revised out loud if it changes
If nothing has shifted by session three or four — That is our signal to rethink the plan or send you back to a doctor. It is not a reason to book you six more
We don't sell blocks up front — Pay as you come. A package of ten paid in advance is a commercial decision dressed as a treatment plan
Discharge is the goal — A physiotherapist you see forever is a physiotherapist who never finished the job
Pain that has lasted months — Pain is not a simple damage meter. The plan is graded loading, sleep, pacing and getting strong in the positions you avoid
After surgery — Your surgeon's protocol leads and we work inside it. Where we disagree with the timeline we ring them, not you
Returning to sport — Progressed on what you can do — hop tests, strength side-to-side, control under fatigue — rather than on the number of weeks elapsed
Balance and falls — Strength and balance work for older adults. The least glamorous thing on this list and the one with the best evidence behind it

What a session actually includes
A proper assessment
Movement, strength and what you actually need to get back to. The history takes longer than people expect.
Hands-on where it helps
To get you moving more comfortably so the exercises are possible — not as a treatment on its own.
Exercises with video
Filmed with you doing them, sent the same day. Nobody remembers a sheet of stick figures by Tuesday.
A load plan
How much, how often, and what to do when it flares. Flares are expected and they are not a setback.
Letters to your doctor
On request, or straight away when something should be looked at by them instead.
Space to work
A rehab area with real weights, because tissue changes need load, not a resistance band on a plinth.
Bladder or bowel changes — With back pain, or numbness around the saddle area. This one is urgent — same day
Sudden weakness — A limb giving way, a foot dragging, or pins and needles spreading rather than settling
After a significant fall — Especially with osteoporosis or on steroids. An x-ray may be the sensible first step
Chest pain, breathlessness, a calf that is hot and swollen — Emergency services. Do not wait for an appointment with anyone
